			<description>&lt;p&gt;&lt;u&gt;&lt;p&gt;PURPOSE OF THE POST&lt;/p&gt;&lt;/u&gt;To support the objectives of our client in meeting the needs of local, single homeless men and women. &lt;p&gt;More specifically:&lt;/p&gt;&lt;p&gt;As part of the Mental Health Team, to assist in the support and resettlement of homeless people, or those vulnerable to homelessness and who appear to have a mental health problem.&lt;/p&gt;&lt;p&gt;There does not need to be a diagnosed mental illness and the individual does not need to be using the existing mental health services. &lt;/p&gt;&lt;u&gt;&lt;p&gt;DUTIES AND RESPONSIBILITIES&lt;/p&gt;&lt;/u&gt;&lt;li&gt; The service:&lt;/p&gt;&lt;li&gt;1 to make assessments of potential service users&amp;rsquo; housing &amp;amp; support needs, where appropriate together with other Service and/or external agency staff and where there appears to be a mental health problem&lt;/p&gt;&lt;li&gt;2 to draw up, implement and regularly review appropriate service user support plans using a case management approach or&lt;/p&gt;&lt;li&gt;3 to, where possible, refer people on to other service staff or external agencies to obtain whatever support seems appropriate and acceptable to the client&lt;/p&gt;&lt;li&gt;4 to assist service users in finding solutions to their social and housing problems to enable them to live as independently as possible in comfort by:-&lt;/p&gt;&lt;p&gt;. meeting with service users at times and locations the suit users needs&lt;/p&gt;&lt;p&gt;. helping service users access services appropriate to their needs&lt;/p&gt;&lt;p&gt;. providing information and advice on welfare rights, housing options, tenancy rights and responsibiliites &lt;/p&gt;&lt;p&gt;. provide information and assistance to service users to enable them to make informed choices about the development of their social network and participation in the local community.&lt;/p&gt;&lt;p&gt;. work with service users to help them learn ,or improve, their practical like skills (eg budgeting, paying bills, planning meals) needed for living as independently as possible&lt;/p&gt;&lt;li&gt;5 to develop and maintain links statutory and voluntary agencies on behalf of service users&lt;/p&gt;&lt;li&gt;6 to keep close contact with statutory mental health services &lt;/p&gt;&lt;li&gt;7 to raise awareness within other agencies of the needs of the homeless mentally ill.&lt;/p&gt;&lt;li&gt;8 to offer assistance to other service staff/projects who require advice or support with clients because of their mental health&lt;/p&gt;&lt;li&gt; Administration:&lt;/p&gt;&lt;li&gt;1 To ensure that all client records are kept in a proper manner and up to date using the system adopted by the service.&lt;/p&gt;&lt;li&gt;2 To maintain, develop and present statistical information relevant to the service for monitoring and evaluation purposes.&lt;/p&gt;&lt;ol&gt;&lt;li&gt; &lt;ol&gt;&lt;li&gt;To be accountable for any expenditure.&lt;/li&gt;&lt;li&gt;To assist in the compilation of an annual report for the use of the service and funding agencies&lt;/li&gt;&lt;/ol&gt;&lt;/li&gt;&lt;/ol&gt;&lt;li&gt; Other&lt;/p&gt;&lt;li&gt;1 To be familiar with and abide by the services polices and procedures&lt;/p&gt;&lt;li&gt;2 Actively promote good practice in respect to equal opportunities, confidentiality and staff safety.&lt;/p&gt;&lt;li&gt;3 To attend Team meetings and other meetings as required.&lt;/p&gt;&lt;li&gt;4 To participate in regular supervision sessions with your line manager.&lt;/p&gt;&lt;li&gt;5 To attend training courses where appropriate.&lt;/p&gt;&lt;li&gt;6 To contribute to discussions about the development of the service.&lt;/p&gt;&lt;li&gt;7 Carry out such duties as may, from time to time, be agreed with your line manager.&lt;/p&gt; &lt;/p&gt;&lt;p&gt;&lt;b&gt;Location:&lt;/b&gt; Cambridge&lt;br /&gt;&lt;b&gt;Salary:&lt;/b&gt; &amp;#163;24,000 - &amp;#163;24,400 pa &lt;br /&gt;&lt;b&gt;Hours:&lt;/b&gt; Full-time&lt;br /&gt;&lt;b&gt;Type:&lt;/b&gt; Permanent&lt;br /&gt;&lt;b&gt;Starting:&lt;/b&gt; Immediately&lt;br /&gt;&lt;b&gt;Duration:&lt;/b&gt; Ongoing&
