We are recruiting on behalf of a South London borough for a Case co-ordinator within an adults and community team.

To assess the needs of clients requiring the services offered by the Council’s Social Services Department and where necessary, in partnership with Health colleagues.

To undertake home visits in order to carry out Contact and/or Overview assessments.

To identify appropriate resources relating to the client’s needs.

To assess for, design and arrange for the delivery of care plans for people assessed using the appropriate assessment tools and guidance.

To undertake, where necessary assessments with other Social Services staff and health care professionals, attend hospital discharge meetings and to participate in case conferences and care planning meetings as required.

To initiate packages of care (within established financial limits) and provide on-going co-ordination and care planning.

To maintain financial awareness in relation to the delivery of care packages with a view to value for money being delivered.

To ensure all financial assessments in respect of all care packages in the community are notified to finance officers as appropriate. To forward all relevant documentation to finance officers as necessary, and appropriately advise service users where to receive benefit advice.

Pay rate is up to £20 per hour.

GSCC Registration is essential.
