			<description>&lt;p&gt;We are recruiting on behalf of a South London borough for a Case co-ordinator within an adults and community team.&lt;li&gt;To assess the needs of clients requiring the services offered by the Council&amp;#146;s Social Services Department and where necessary, in partnership with Health colleagues. &lt;li&gt;To undertake home visits in order to carry out Contact and/or Overview assessments. &lt;li&gt;To identify appropriate resources relating to the client&amp;#146;s needs.&lt;li&gt;To assess for, design and arrange for the delivery of care plans for people assessed using the appropriate assessment tools and guidance. &lt;li&gt;To undertake, where necessary assessments with other Social Services staff and health care professionals, attend hospital discharge meetings and to participate in case conferences and care planning meetings as required.&lt;li&gt;To initiate packages of care (within established financial limits) and provide on-going co-ordination and care planning.&lt;li&gt;To maintain financial awareness in relation to the delivery of care packages with a view to value for money being delivered. &lt;li&gt;To ensure all financial assessments in respect of all care packages in the community are notified to finance officers as appropriate.
